Parathyroid hyperplasia

  1. Parathyroid hyperplasia- absolute increase in the mass of the parenchymal cells of the parathyroid gland-most commonly secondary to hypeplasia of the chief cells
  2. Primary hyperparathyroidism is a generalized disorder of Ca, PO4, and bone metabolism due to an increased secretion of PTH; usually leads to hypercalcemia and hypophosphatemia
  3. Seen in MEN I and IIa
  4. Asymptomatic hypercalcemia is the commonest manifestation but about 25% of patients will have evidence of nephrolithiasis or nephrocalcinosis
  5. Frequently involves multiple glands either as diffuse hyperplasia or multiple adenomas
